Insurance and Fees

Free Initial Telephone Consultation

I provide a free 5-10 minute telephone consultation to all prospective patients to ask a few questions and to give you a chance to ask any questions you may have.

“In-Network” Insurance (updated January 4th, 2012)

I am currently an “in-network provider” for:

  • Aetna HMO
  • Aetna PPO
  • Anthem Blue Cross HMO
  • Anthem Blue Cross PPO
  • Most “Blue Cross/Blue Shield” plans
  • Blue Shield of California members where Magellan Health Services serves as its Mental Health Service Administrator
  • Magellan Health Services

Keep in mind that the insurance company that handles your medical insurance may not be the same one that handles your mental health insurance – call the number for mental health on the back of your insurance card to confirm your specific situation.

“Out-of-Network” Insurance (updated January 4th, 2012)

At the current time, I am considered an “out-of-network provider” for any insurance panel not listed above.  If I am an “out-of-network provider” for your insurance, I will provide you with an itemized receipt to submit to insurance for reimbursement.  Ask your insurance company for the reimbursement rates for “out-of-network providers.”

Fees

If I am an “in-network provider” for you, then you would pay applicable copays, coinsurance, and deductibles specific to your insurance plan for outpatient mental health office visits.  If I am a “out-of-network provider”, or if you do not have insurance, please contact me for fees.

Payment

Payment is expected at the time of service.  I accept cash, check, or credit card.
