Second Opinion Consultation

• I am available for second opinion consultation for other clinicians seeking input.  I am also available to conduct second opinion consultations for patients who have questions or concerns about their current treatment.  In these cases, your therapist / psychiatrist’s knowledge of this will be important.

I understand Dr. Mao holds that all information concerning patients is confidential and is released only through

procedures consistent with the law and professional ethics. The law requires the release of confidential

information in situations of potential harm to oneself or others, when records are demanded by court order, or

in instances of suspected child or elder abuse.

I understand that Dr. Mao will be required to submit my medical information to my insurance and prescription

medication plans. I hereby authorize the release of any medical information necessary to process my insurance

and prescription medication claims.
